Full re-assignment surgery as more popular in media is the accepted for extreme cases.  However it can be an issue of low self esteem to permanent gender change which cannot be reveresed but as we there have been horrific stories of 2 or 3 re-assignment surgery per case. Cross dressing is better way by far.
